During system suspend, USB core will let HC suspends the device if it
doesn't have remote wakeup enabled and doesn't have any children.
However, from the log above we can see that the usb 1-7 doesn't get bus
suspended due to not in U0. After a while the port finished U2 -> U0
transition, interrupts the suspend process.

The observation is that after disabling LPM, port doesn't transit to U0
immediately and can linger in U2. xHCI spec 4.23.5.2 states that the
maximum exit latency for USB2 LPM should be BESL + 10us. The BESL for
the affected device is advertised as 400us, which is still not enough
based on my testing result.

So let's use the maximum permitted latency, 10000, to poll for U0
status to solve the issue.

Al Cooper dbf2c1a8a2 xhci: Fix enumeration issue when setting max packet size for FS devices.
commit a73d9d9cfc upstream.

Unable to complete the enumeration of a USB TV Tuner device.

Per XHCI spec (4.6.5), the EP state field of the input context shall
be cleared for a set address command. In the special case of an FS
device that has "MaxPacketSize0 = 8", the Linux XHCI driver does
not do this before evaluating the context. With an XHCI controller
that checks the EP state field for parameter context error this
causes a problem in cases such as the device getting reset again
after enumeration.

When that field is cleared, the problem does not occur.

This was found and fixed by Sasi Kumar.

Mathias Nyman f0a2a8d0ad xhci: Fix incorrect EP_STATE_MASK

EP_STATE_MASK should be 0x7 instead of 0xf

xhci spec 6.2.3 shows that the EP state field in the endpoint context data
structure consist of bits [2:0].
The old value included a bit from the next field which fortunately is a
 RsvdZ region. So hopefully this hasn't caused too much harm

Tang Bin 6119a89ab5 usb: host: ehci-exynos: Fix error check in exynos_ehci_probe()

If the function platform_get_irq() failed, the negative value
returned will not be detected here. So fix error handling in
exynos_ehci_probe(). And when get irq failed, the function
platform_get_irq() logs an error message, so remove redundant
message here.

Longfang Liu 469adc4cd9 USB: ehci: reopen solution for Synopsys HC bug

A Synopsys USB2.0 core used in Huawei Kunpeng920 SoC has a bug which
might cause the host controller not issuing ping.

Bug description:
After indicating an Interrupt on Async Advance, the software uses the
doorbell mechanism to delete the Next Link queue head of the last
executed queue head. At this time, the host controller still references
the removed queue head(the queue head is NULL). NULL reference causes
the host controller to lose the USB device.

Solution:
After deleting the Next Link queue head, when has_synopsys_hc_bug set
to 1,the software can write one of the valid queue head addresses to
the ASYNCLISTADDR register to allow the host controller to get
the valid queue head. in order to solve that problem, this patch set
the flag for Huawei Kunpeng920

The above NULL pointer dereference is the result of block_len and the
sent_len set to zero after the first SG of the list when IOMMU driver
is enabled. Because of this the loop of processing the SGs has run
more than num_sgs which resulted in a sg_next on the last SG of the
list which has SG_END set.

Fix this by check for the sg before any attributes of the sg are
accessed.

Mathias Nyman ed75a2c65b xhci: Force Maximum Packet size for Full-speed bulk devices to valid range.
commit f148b9f402 upstream.

A Full-speed bulk USB audio device (DJ-Tech CTRL) with a invalid Maximum
Packet Size of 4 causes a xHC "Parameter Error" at enumeration.

This is because valid Maximum packet sizes for Full-speed bulk endpoints
are 8, 16, 32 and 64 bytes. Hosts are not required to support other values
than these. See usb 2 specs section 5.8.3 for details.

The device starts working after forcing the maximum packet size to 8.
This is most likely the case with other devices as well, so force the
maximum packet size to a valid range.

Erkka Talvitie b195c3ffe4 USB: EHCI: Do not return -EPIPE when hub is disconnected
commit 64cc3f12d1 upstream.

When disconnecting a USB hub that has some child device(s) connected to it
(such as a USB mouse), then the stack tries to clear halt and
reset device(s) which are _already_ physically disconnected.

The issue has been reproduced with:

CPU: IMX6D5EYM10AD or MCIMX6D5EYM10AE.
SW: U-Boot 2019.07 and kernel 4.19.40.

CPU: HP Proliant Microserver Gen8.
SW: Linux version 4.2.3-300.fc23.x86_64

In this situation there will be error bit for MMF active yet the
CERR equals EHCI_TUNE_CERR + halt. Existing implementation
interprets this as a stall [1] (chapter 8.4.5).

The possible conditions when the MMF will be active + halt
can be found from [2] (Table 4-13).

Fix for the issue is to check whether MMF is active and PID Code is
IN before checking for the stall. If these conditions are true then
it is not a stall.

What happens after the fix is that when disconnecting a hub with
attached device(s) the situation is not interpret as a stall.

Mathias Nyman d93f4bce1c xhci: fix USB3 device initiated resume race with roothub autosuspend
commit 057d476fff upstream.

A race in xhci USB3 remote wake handling may force device back to suspend
after it initiated resume siganaling, causing a missed resume event or warm
reset of device.

When a USB3 link completes resume signaling and goes to enabled (UO)
state a interrupt is issued and the interrupt handler will clear the
bus_state->port_remote_wakeup resume flag, allowing bus suspend.

If the USB3 roothub thread just finished reading port status before
the interrupt, finding ports still in suspended (U3) state, but hasn't
yet started suspending the hub, then the xhci interrupt handler will clear
the flag that prevented roothub suspend and allow bus to suspend, forcing
all port links back to suspended (U3) state.

Example case:
usb_runtime_suspend() # because all ports still show suspended U3

    hub_suspend();   # successful as hub->wakeup_bits not set yet

    clear bus_state->port_remote_wakeup
    usb_wakeup_notification()
      sets hub->wakeup_bits;
        kick_hub_wq()

      hcd_bus_suspend()
        xhci_bus_suspend() # success as port_remote_wakeup bits cleared

Fix this by increasing roothub usage count during port resume to prevent
roothub autosuspend, and by making sure bus_state->port_remote_wakeup
flag is only cleared after resume completion is visible, i.e.
after xhci roothub returned U0 or other non-U3 link state link on a
get port status request.

Mathias Nyman bedf0c0e55 xhci: make sure interrupts are restored to correct state

spin_unlock_irqrestore() might be called with stale flags after
reading port status, possibly restoring interrupts to a incorrect
state.

If a usb2 port just finished resuming while the port status is read
the spin lock will be temporary released and re-acquired in a separate
function. The flags parameter is passed as value instead of a pointer,
not updating flags properly before the final spin_unlock_irqrestore()
is called.

Kai-Heng Feng a656cabbe0 xhci: Increase STS_SAVE timeout in xhci_suspend()

After commit f7fac17ca9 ("xhci: Convert xhci_handshake() to use
readl_poll_timeout_atomic()"), ASMedia xHCI may fail to suspend.

Although the algorithms are essentially the same, the old max timeout is
(usec + usec * time of doing readl()), and the new max timeout is just
usec, which is much less than the old one.

Increase the timeout to make ASMedia xHCI able to suspend again.

Jan Schmidt 6e29e0924c xhci: Check all endpoints for LPM timeout
commit d500c63f80 upstream.

If an endpoint is encountered that returns USB3_LPM_DEVICE_INITIATED, keep
checking further endpoints, as there might be periodic endpoints later
that return USB3_LPM_DISABLED due to shorter service intervals.

Without this, the code can set too high a maximum-exit-latency and
prevent the use of multiple USB3 cameras that should be able to work.

Mathias Nyman 0ccae3031c xhci: Prevent device initiated U1/U2 link pm if exit latency is too long

If host/hub initiated link pm is prevented by a driver flag we still must
ensure that periodic endpoints have longer service intervals than link pm
exit latency before allowing device initiated link pm.

Fix this by continue walking and checking endpoint service interval if
xhci_get_timeout_no_hub_lpm() returns anything else than USB3_LPM_DISABLED

While at it fix the split line error message

Mathias Nyman 1817e62b77 xhci: Fix false warning message about wrong bounce buffer write length
commit c03101ff4f upstream.

The check printing out the "WARN Wrong bounce buffer write length:"
uses incorrect values when comparing bytes written from scatterlist
to bounce buffer. Actual copied lengths are fine.

The used seg->bounce_len will be set to equal new_buf_len a few lines later
in the code, but is incorrect when doing the comparison.

The patch which added this false warning was backported to 4.8+ kernels
so this should be backported as far as well.

ohci_shutdown() disables all the interrupt and rh_state is set to
OHCI_RH_HALTED. In other hand, ohci_irq() is possible to enable
OHCI_INTR_SF and OHCI_INTR_MIE on ohci_irq(). Note that OHCI_INTR_SF
is possible to be set by start_ed_unlink() which is called:
 ohci_irq()
  -> process_done_list()
   -> takeback_td()
    -> start_ed_unlink()

So, ohci_irq() has the following condition, the issue happens by
&ohci->regs->intrenable = OHCI_INTR_MIE | OHCI_INTR_SF and
ohci->rh_state = OHCI_RH_HALTED:

To fix the issue, ohci_shutdown() holds the spin lock while disabling
the interruption and changing the rh_state flag to prevent reenable
the OHCI_INTR_MIE unexpectedly. Note that io_watchdog_func() also
calls the ohci_shutdown() and it already held the spin lock, so that
the patch makes a new function as _ohci_shutdown().

This patch is inspired by a Renesas R-Car Gen3 BSP patch
from Tho Vu.

Ryan Kennedy 71960676d4 usb: pci-quirks: Correct AMD PLL quirk detection
commit f3dccdaade upstream.

The AMD PLL USB quirk is incorrectly enabled on newer Ryzen
chipsets. The logic in usb_amd_find_chipset_info currently checks
for unaffected chipsets rather than affected ones. This broke
once a new chipset was added in e788787ef. It makes more sense
to reverse the logic so it won't need to be updated as new
chipsets are added. Note that the core of the workaround in
usb_amd_quirk_pll does correctly check the chipset.

The failing case is when disabling LPM. So it is sufficient to avoid

Andrey Smirnov 307031c53c xhci: Convert xhci_handshake() to use readl_poll_timeout_atomic()
commit f7fac17ca9 upstream.

Xhci_handshake() implements the algorithm already captured by
readl_poll_timeout_atomic(). Convert the former to use the latter to
avoid repetition.

Turned out this patch also fixes a bug on the AMD Stoneyridge platform
where usleep(1) sometimes takes over 10ms.
This means a 5 second timeout can easily take over 15 seconds which will
trigger the watchdog and reboot the system.

Henry Lin fa2fc3c854 xhci: update bounce buffer with correct sg num
commit 597c56e372 upstream.

This change fixes a data corruption issue occurred on USB hard disk for
the case that bounce buffer is used during transferring data.

While updating data between sg list and bounce buffer, current
implementation passes mapped sg number (urb->num_mapped_sgs) to
sg_pcopy_from_buffer() and sg_pcopy_to_buffer(). This causes data
not get copied if target buffer is located in the elements after
mapped sg elements. This change passes sg number for full list to
fix issue.

Besides, for copying data from bounce buffer, calling dma_unmap_single()
on the bounce buffer before copying data to sg list can avoid cache issue.

Mathias Nyman 2423480fcd xhci: Fix port resume done detection for SS ports with LPM enabled
commit 6cbcf59693 upstream.

A suspended SS port in U3 link state will go to U0 when resumed, but
can almost immediately after that enter U1 or U2 link power save
states before host controller driver reads the port status.

Host controller driver only checks for U0 state, and might miss
the finished resume, leaving flags unclear and skip notifying usb
code of the wake.

Add U1 and U2 to the possible link states when checking for finished
port resume.

The function r8a66597_endpoint_disable() and r8a66597_urb_enqueue() may
be concurrently executed.
The two functions both access a possible shared variable "hep->hcpriv".

This shared variable is freed by r8a66597_endpoint_disable() via the
call path:

  kfree(hep->hcpriv) (line 1995 in Linux-4.19)

This variable is read by r8a66597_urb_enqueue() via the call path:
r8a66597_urb_enqueue
  spin_lock_irqsave(&r8a66597->lock)
  init_pipe_info
    enable_r8a66597_pipe
      pipe = hep->hcpriv (line 802 in Linux-4.19)

The read operation is protected by a spinlock, but the free operation
is not protected by this spinlock, thus a concurrency use-after-free bug
may occur.

To fix this bug, the spin-lock and spin-unlock function calls in
r8a66597_endpoint_disable() are moved to protect the free operation.

Mathias Nyman e953031c6d xhci: Don't prevent USB2 bus suspend in state check intended for USB3 only
commit 45f750c16c upstream.

The code to prevent a bus suspend if a USB3 port was still in link training
also reacted to USB2 port polling state.
This caused bus suspend to busyloop in some cases.
USB2 polling state is different from USB3, and should not prevent bus
suspend.

Limit the USB3 link training state check to USB3 root hub ports only.
The origial commit went to stable so this need to be applied there as well

Mathias Nyman d65afda688 xhci: Prevent U1/U2 link pm states if exit latency is too long
commit 0472bf06c6 upstream.

Don't allow USB3 U1 or U2 if the latency to wake up from the U-state
reaches the service interval for a periodic endpoint.

This is according to xhci 1.1 specification section 4.23.5.2 extra note:

"Software shall ensure that a device is prevented from entering a U-state
 where its worst case exit latency approaches the ESIT."

Allowing too long exit latencies for periodic endpoint confuses xHC
internal scheduling, and new devices may fail to enumerate with a
"Not enough bandwidth for new device state" error from the host.

Aaron Ma 0c9aa4da45 usb: xhci: fix uninitialized completion when USB3 port got wrong status

Realtek USB3.0 Card Reader [0bda:0328] reports wrong port status on
Cannon lake PCH USB3.1 xHCI [8086:a36d] after resume from S3,
after clear port reset it works fine.

Since this device is registered on USB3 roothub at boot,
when port status reports not superspeed, xhci_get_port_status will call
an uninitialized completion in bus_state[0].
Kernel will hang because of NULL pointer.

Restrict the USB2 resume status check in USB2 roothub to fix hang issue.

Mathias Nyman 1a8174028d usb: xhci: Prevent bus suspend if a port connect change or polling state is detected
commit 2f31a67f01 upstream.

USB3 roothub might autosuspend before a plugged USB3 device is detected,
causing USB3 device enumeration failure.

USB3 devices don't show up as connected and enabled until USB3 link trainig
completes. On a fast booting platform with a slow USB3 link training the
link might reach the connected enabled state just as the bus is suspending.

If this device is discovered first time by the xhci_bus_suspend() routine
it will be put to U3 suspended state like the other ports which failed to
suspend earlier.

The hub thread will notice the connect change and resume the bus,
moving the port back to U0

This U0 -> U3 -> U0 transition right after being connected seems to be
too much for some devices, causing them to first go to SS.Inactive state,
and finally end up stuck in a polling state with reset asserted

Fix this by failing the bus suspend if a port has a connect change or is
in a polling state in xhci_bus_suspend().

Don't do any port changes until all ports are checked, buffer all port
changes and only write them in the end if suspend can proceed

Mathias Nyman 52f305530a xhci: Fix USB3 NULL pointer dereference at logical disconnect.

Hub driver will try to disable a USB3 device twice at logical disconnect,
racing with xhci_free_dev() callback from the first port disable.

This can be triggered with "udisksctl power-off --block-device <disk>"
or by writing "1" to the "remove" sysfs file for a USB3 device
in 4.17-rc4.

USB3 devices don't have a similar disabled link state as USB2 devices,
and use a U3 suspended link state instead. In this state the port
is still enabled and connected.

hub_port_connect() first disconnects the device, then later it notices
that device is still enabled (due to U3 states) it will try to disable
the port again (set to U3).

The xhci_free_dev() called during device disable is async, so checking
for existing xhci->devs[i] when setting link state to U3 the second time
was successful, even if device was being freed.

The regression was caused by, and whole thing revealed by,
Commit 44a182b9d1 ("xhci: Fix use-after-free in xhci_free_virt_device")
which sets xhci->devs[i]->udev to NULL before xhci_virt_dev() returned.
and causes a NULL pointer dereference the second time we try to set U3.

Fix this by checking xhci->devs[i]->udev exists before setting link state.

The original patch went to stable so this fix needs to be applied there as
well.

Mathias Nyman d5b33599b3 xhci: Don't print a warning when setting link state for disabled ports

When disabling a USB3 port the hub driver will set the port link state to
U3 to prevent "ejected" or "safely removed" devices that are still
physically connected from immediately re-enumerating.

If the device was really unplugged, then error messages were printed
as the hub tries to set the U3 link state for a port that is no longer
enabled.

xhci-hcd ee000000.usb: Cannot set link state.
usb usb8-port1: cannot disable (err = -32)

Don't print error message in xhci-hub if hub tries to set port link state
for a disabled port. Return -ENODEV instead which also silences hub driver.
